We report a case of Primary Delusional Disorder of Infestation, although most patients reject psychiatric referral and are difficult to engage in medical care, this patient complaint well with medical indications and treatment, probably because the obsessive personality traits. He had mild favorable response to the pharmacological management with quetiapine.
